Another Clutch question

My 91 hardbody 5 speed, with 47000 miles has good pickup in 1st and 2nd gear. By 3rd gear, I lift off the clutch, and seem to get power, but as I give additional gas with the accelerator, the RPMs rev without additional power. I let off the gas and the clutch grabs again.

Is this a warn out clutch, or a clutch in need of adjustment, or could it be something else.

My 17 year old son learned to drive with this truck.
