lose my wheel second time !!!

hello i lost the wheel from my 98 ecoline again for the the second time ! the car is in good state .it used to be a bus for cariying passengers but i take out the backseat and use it for commercial purpose .i dont overload it and i even made an extra flatspring to carry more weight .two times the wheel bounced to the other side of the highway in between the cars and end up in the hard shoulder without making damage to the other cars!! does someone have a cloe what happend .before the wheel goes of the car feels like a get a flat tire and i checked it but everything was ok .does someone have this expierince before and can you tel me what to do about it thanks Nico
